Her urinating vulva is explicitly shown as she urinates, and her urination is obviously real. An interview with a film crew man confirmed that the urination of the young woman is real, as if it isn't immediately obvious. “We have this scene where Maggie Gyllenhaal just reads her lines straight into the camera, and in the background two of them are eating each other’s asses out,” he said. “We also have this scene where one of them just pees, just urinates right on a table. I was giving her water for hours, and then she just had to hold it until we said ‘go.’” There are likely other urination and (probably faked) scatological scenes in the film as well.

There are two male pee scenes. I checked and the first (around an hour in) just shows a stream, while the second (around two and a half hours) seems to be real. There is also a lot of scat in the movie, but it all looks faked.
