A lot of video conference programs can be set to automatically record. It's useful if you're watching someone give a presentation, and you want to keep a copy of it to refer to later for information.

As for the video getting out, that simply means that someone with a recorded copy posted it somewhere. This could have been one of any unknown number of people on the call, not just the 11 we see using their webcams.

Maybe it's a bit unusual for people to get caught using a bathroom during video conferences, but many other thing get exposed as well on the same way. Like people wearing unappropriated clothes or lack of clothes or some other things people share unintentionally while on their web cams. One of my friend who is also a crossdresser exposed himself like that on video call in front of his coworkers while working from home. He was wearing male clothes only on upper part of his body. Below that he was wearing mini skirt, fishnet stockings and high heels. He didn't end video call before he got up after the meeting was over. Everyone could see him wearing female sexy clothes on lower part of his body. He didn't intend to expose himself like that, but momentary he forgot about the camera and got up. He is "out of the closet" now and divorced, because gradually everyone find out about what happened, even his wife. It was not published on the internet or anything, but people talk. And he still have to wear male clothes at work, even they know he is a crossdresser. And he believes that couple of seconds in front of camera hurt his career long term even nobody explicitly said that.

I thought this would be some stupid, unrealistic idea for a porn video. Poor gal 😄 .

I assume you couldn't see anything private because the video seems to be referenced to on a regular site, not a porn site (I get an error message watching it so maybe the video is down). Hopefully it'll just be a friendly running joke in the office and not something she's embarrassed about for a long time 😛 .

No, we couldn't see much. But it was clear what was going on.

First, this was obviously on a cell phone, not a computer / webcam.

 

This is literally my field of expertise (video / web/ whatever- conferencing) and regularly work with both Zoom and WebEx (Cisco's version) even prior to the covid-pocalypse. Its surprisingly easy to do if you aren't careful. I myself was walking around nude last week, but luckily only showed my upper body before I realized it.

The reason is that a lot of people join on their phones, and these programs automatically share your camera by default, as its assumed you are joining the meeting to see each other other. You would assume that by looking at other people you would realize that you are also being shown, but it becomes second nature very quickly. Also, if someone else is presenting something it makes it even easier to forget you are on camera. People bring their phones everywhere with them when they are on phone conferences, but it doesn't matter because they are on mute.

Yes, generally speaking all of these programs automatically record everything. Its very useful for people who couldn't join the call, or just archival purposes.
